Elimination & Food Sensitivity Journal

A structured trial-and-error log to track dietary triggers, identify GI and systemic reactions, and bring data-backed evidence to gastroenterology and allergy consultations.

Certain foods can trigger or worsen physical symptoms such as abdominal pain, severe cramping, nausea, diarrhea, skin flushing, or fatigue. Finding them usually takes systematic trial and error: log each new food you introduce and any reaction that follows.

What each column is for
  • Date and timeWhen the food was tried.
  • Food and ingredientsBe specific: "dairy milk in oatmeal," not "breakfast."
  • Portion sizeHalf a cup, one slice, a handful.
  • Symptoms and reactionsSeverity from 1 to 10, how long until it started, and how long it lasted.
  • Notes and possible triggersGI pain, hives, a fatigue flare, and what you had to do about it.
A filled-in row

10/14, 12:30 PM. Dairy milk in oatmeal, half a cup. Bloating and severe cramping, 7 out of 10, starting 30 minutes after eating. Needed a heating pad and missed an afternoon class.

What to say at a dietitian or GI appointment

I have been using this structured food journal to log my dietary trials and physical responses. As you can see, when I reintroduce (food or ingredient), I experience reproducible physical symptoms within (X) hours, which suggests a food intolerance or motility issue rather than generalized functional stress.
